HOUSING CHART AND BEDDING NEEDED — HERE This is final
BORDER ID INFO FOR US CITIZENS
1. Age 16 or over need EITHER a Passport OR [VT, NY, MI, MN residents only- an ENHANCED ID] — see below; different states differ on what they issue
- There is a difference between state issued ID forms depending on what state you live in. Your REAL ID Driver License or Non-Driver Identification Card will be marked with a star in the top right corner if to show it is a Federally approved “Real ID,” In VT, NY, MI, and MN they issue a combination Real ID/enhanced card that is good for land border crossings. This is not true of other states. [updated 2024] Here is some New Hampshire info.

you can read more about it on the TSA website linked above — click on your state to see your specifics. Feel free to ask questions of me! Sorry, this can be confusing because there are differences for each state, and some states use the same name of ID for different meanings!!
2. Age 15 or below need one of the below:
- REAL [embossed] Birth Certificate OR
- “Real ID” non-driver ID from your state DMV OR
- Military ID – for dependents of US Military
3. ADDITIONAL REQUIREMENT:

ALL team members under age 18 who are not traveling with BOTH parents need this NOTARY CONSENT FORM signed by both parents and notarized: 2024 FORM

TRAVEL DETAILS — SCHEDULE FOR LOADING AND TRAVELING HERE We are traveling to arrive SATURDAY JUNE 29TH, first meal to be Dinner at 6 PM. Please let Olivia know if you plan to arrive much earlier that that.
THESE TWO FORMS MUST BE SENT TO OLIVIA BY EACH TEAM MEMBER PRIOR TO DEPARTURE DAY.
- E4K MEDICAL INSURANCE INFORMATION FORM – Everyone must sign [adults for themselves and all teens under 18] must have their parents sign before leaving for Quebec. Sign, scan/email OR take a picture and email to Olivia@copcvt.org.
2. E4K MEDICAL PERMISSION AND LIABILITY RELEASE FORM — Everyone must sign [adults for themselves and all teens under 18] must have their parents sign before leaving for Quebec. Sign, scan or take a picture and send to Olivia@copcvt.org.
Deadline for sending copies of the Notary form and the two Medical forms above is June 19th! Scan or photo and Email to Olivia please.

Quebec History
Here are links to a talk given by Pastor Ben Westerveld about the history of Quebec, the evangelical church in Quebec, and how the camps fit into the church’s work there. You might find it very interesting to listen to but it’s not mandatory. We filmed this, I believe, in 2010 in Quebec City.
